Online appointment booking for a multi-doctor clinic
A booking system where patients pick a doctor and a free slot in under a minute, and clinic staff manage the day, the week and patient reminders from one dashboard.
Demo project: the business shown is fictional. We built this working product to show how we would solve this problem for a real client in this industry.
The problem
Most small clinics in Indore still run appointments on a phone and a paper register. The front desk spends its day answering calls, patients wait on hold or turn up to long queues, and nobody can see at a glance how busy the coming week is. No-shows are common because nobody has time to call patients the day before.
What we built
For patients
- Choose a doctor, see their real free slots for the next two weeks, and book in about 60 seconds without creating an account.
- Get an instant booking reference, share the appointment on WhatsApp, and cancel online (verified by the mobile number used to book) so the slot frees up for someone else.
For the clinic
- A dashboard showing today’s patients, upcoming load, completed visits and the cancellation/no-show rate.
- A filterable appointment list (by date, status, name, phone or reference) with one-click status updates.
- A week calendar per doctor.
- A WhatsApp reminder button that opens a pre-written message to the patient and records it in a reminder log.
How it works
- Database-level guarantees. A partial unique index on doctor, date and time makes double booking impossible, even if two patients click at the same moment. Booking runs through a single Postgres function that validates the doctor’s working days, hours and slot length, so the rules cannot be bypassed from the browser.
- Security by design. Patients never get read access to the appointments table. Row-level security lets admins see every doctor while each doctor sees only their own patients.
- India-first details. All dates and times are computed in IST no matter where the server runs, phone numbers accept Indian formats, and reminders go through WhatsApp, which is how patients actually communicate.
- Tested. Unit tests cover slot generation and time-zone handling, and SQL tests in CI verify booking, double-booking protection, cancellation and role isolation.

Taking it to production
For a real clinic we would add automated reminders through the WhatsApp Business API, online fee collection with Razorpay, SMS OTP verification, patient history and prescriptions, and multi-branch support. A typical build is 5 to 7 weeks.
